  1. 4 copies Personal Data Sheet
  2. 4 copies Performance Description Form
  3. 4 copies Statement of Assets and Liabilities and Networth
  4. Authenticated PRC ID
  5. Authenticated PRC Board Rating
  6. Updated NBI clearance
  7. Other requirements maybe submitted once accepted.
